Job summary

Lifepoint Rehabilitation seeks a Clinical Liaison to connect referring facilities with our hospital admissions teams. You will assess patient needs, coordinate intake and pre-admission screening, and determine admission appropriateness with on-site clinical judgment.

You will communicate options to patients and families and secure information on resources and benefits. Applicants should hold clinical licensure (RN, PT, OT, SLP), have EMR experience, a valid driver's license, and strong

Experience in clinical assessment and EMR systems.
Valid driver's license.

Responsibilities

Builds and maintains referral relationships to drive appropriate admissions to the hospital
Assist and coordinate the intake and pre-admission screening process
Perform on-site clinical judgment to determine admission appropriateness
Communicates rehabilitation and facility options to patients and families; assesses expectations and requirements for participation
Secures information relating to patients' resources and benefits
Functions as initial contact for external case management and discharge planner personnel with payors and hospital facilities
