Overview
Clinical Liaison (RN / RRT) at Select Specialty Hospital, a division of Select Medical, focused on Critical Illness Recovery Hospital (LTACH) settings. This role supports referred patients in pre-admission processes by collaborating with clinical teams to assess patient needs and serves as a resource for healthcare professionals through fostering relationships within medical facilities.
Responsibilities
- Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
- Coordinate care plans with physicians and families using clinical expertise and problem-solving skills.
- Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
- Work with physicians, discharge planners and patient families to plan the best continuum of care after ICU or post-surgical stays by utilizing strong clinical background, payer knowledge and problem-solving skills.
- Partner with hospital case managers and physicians to create smooth pre-admissions and transfer processes, removing payer-related and operational barriers.
Qualifications
- Required: Registered Nurse (RN) or Respiratory Therapist (RT) license.
- Other designations (e.g., LPN, PT, OT) considered upon further review.
- Ability to complete clinical assessments of medically complex patients.
Preferred Skills
- Demonstrated success in patient advocacy.
- Superior communication and relationship-building skills.
- Prior healthcare marketing or business development experience is preferred, but extensive training is provided.
