Job summary

Vantage Care Management seeks a Clinical Liaison to bridge our centers with hospital partners and to guide patients and families through the next steps in care. You will work with a supportive team to promote our mission and services while driving admissions with strategic outreach.

In this role, you will build relationships with medical professionals, attend meetings, manage referral logs, and deliver end-of-day reports.

Qualifications

• Valid driver's license and willingness to travel.
• RN, LPN, or LSW preferred but not required.
• Ability to manage multiple residents and referrals in a fast-paced environment.

Responsibilities

• Build and maintain strong relationships with medical professionals, hospitals, and community healthcare partners.
• Represent and promote the organization's mission, philosophy, and services both within hospitals and throughout the community.
• Attend facility meetings, calls, and educational in-services.
• Maintain accurate and up-to-date referral tracking logs.
• Provide timely end-of-day reports to department heads.
