Job summary

Select Specialty Hospital - Miami Lakes is seeking a Clinical Liaison (RN or RT preferred) to advocate for referred patients during pre-admission by collaborating with clinical teams. You will coordinate care plans with physicians, families, and case managers to smooth transfers and overcome access barriers to post-ICU care.

Full-time schedule is Sunday-Thursday, 8 AM–5 PM, with a generous monthly bonus and an extensive onboarding/training program.

Qualifications

• RN or RT license required or equivalent.
• Ability to complete clinical assessments of medically complex patients.
• Bilingual English/Spanish preferred.

Responsibilities

• Work with physicians, discharge planners, and families to plan the continuum of care after ICU or surgery.
• Perform clinical assessments of referred patients and counsel families on care decisions.
• Coordinate pre-admission and transfers by removing payer-related and operational barriers.
• Build relationships with healthcare facilities to facilitate patient access to post-ICU care.

A day in the life

You’ll advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills, you’ll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.

Key Responsibilities

• You will work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge, and problem-solving skills.
• Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.

Required Skills

• Registered Nurse (RN) or Respiratory Therapist (RT) License. Will consider other designations like LPN, PT, OT, etc., upon further review.
• Ability to complete clinical assessments of medically complex patients.

Preferred skills that will make you a successful Clinical Liaison:

• Demonstrated success in patient advocacy.
• Superior communication and relationship-building skills.
• Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.
• Bilingual (English/Spanish)
