Job summary

Qualifications

• Previous experience as a Clinical Liaison or Hospital Liaison in skilled nursing or long-term care.
• Experience with hospitals, physicians, case managers, discharge planners, and referral sources.
• Knowledge of skilled nursing, rehabilitation, long-term care, or post-acute healthcare services.
• Strong clinical assessment and communication skills.

Aventura at Carriage is seeking a compassionate, knowledgeable, and relationship-focused Clinical Liaison to help connect hospitals, healthcare providers, residents, and families with our skilled nursing and rehabilitation services.

The Clinical Liaison will play an important role in developing referral relationships, supporting the admissions process, and helping ensure a smooth transition from hospital to skilled nursing care.

What You'll Do

• Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, discharge planners, and other healthcare professionals.
• Serve as a professional representative of the facility within hospitals and the local healthcare community.
• Conduct regular hospital rounds and maintain consistent communication with referral sources.
• Identify potential skilled nursing and rehabilitation referrals.
• Respond promptly to referral opportunities and work closely with the admissions and intake teams.
• Complete clinical assessments and bedside evaluations when appropriate.
• Assist residents and families in understanding their post-acute care options.
• Communicate clinical information and admission updates to referral sources in a timely manner.
• Follow referrals through the admissions process and maintain consistent follow-up.
• Build awareness of the facility's services, clinical capabilities, and specialty programs.
• Participate in educational presentations, provider meetings, community events, and outreach activities.
• Maintain accurate referral and marketing activity in the appropriate CRM/EHR systems.
• Identify barriers to referrals and communicate trends and opportunities to facility leadership.
• Collaborate with the interdisciplinary team to support census development and successful transitions.

What We're Looking For

• Previous experience as a Clinical Liaison, Hospital Liaison, Skilled Nursing Admissions Coordinator, or Healthcare Marketer within a skilled nursing or long-term care setting.
• Experience working with hospitals, physicians, case managers, discharge planners, and referral sources.
• Knowledge of skilled nursing, rehabilitation, long-term care, or post-acute healthcare services.
• Strong clinical assessment and communication skills.
• Excellent relationship-building, presentation, and networking abilities.
• Professional, compassionate, and customer-focused approach.
• Strong organizational and time-management skills.
• Ability to work independently while collaborating with clinical and administrative teams.
• Proficiency with Microsoft Office, CRM, and/or EHR systems.
• Valid driver's license and reliable transportation for hospital and community outreach.
