Aventura at Carriage Inn in Dayton, OH seeks a Clinical Liaison to connect hospitals, physicians, and long-term care teams with our skilled nursing and rehabilitation services. You will develop referral relationships, support admissions, and guide transitions from hospital to post-acute care, ensuring timely communications and smooth patient handoffs.

Ideal candidates have healthcare marketing or liaison experience, strong relationships, and excellent communication skills.

Qualifications

• Experience as Clinical Liaison or similar in skilled nursing or long-term care.
• Experience working with hospitals, physicians, case managers, discharge planners, and referral sources.
• Knowledge of skilled nursing, rehabilitation, long-term care, or post-acute healthcare services.
• Strong clinical assessment and communication skills.
• Excellent relationship-building, presentation, and networking abilities.
• Professional, compassionate, and customer-focused approach.
• Strong organizational and time-management skills.
• Ability to work independently while collaborating with clinical and administrative teams.
• Proficiency with Microsoft Office, CRM, and/or EHR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, discharge planners, and other healthcare professionals.
• Serve as a professional representative of the facility within hospitals and the local healthcare community.
• Conduct regular hospital rounds and maintain consistent communication with referral sources.
• Identify potential skilled nursing and rehabilitation referrals.
• Respond promptly to referral opportunities and work closely with the admissions and intake teams.
• Complete clinical assessments and bedside evaluations when appropriate.
• Assist residents and families in understanding their post-acute care options.
• Communicate clinical information and admission updates to referral sources in a timely manner.
• Follow referrals through the admissions process and maintain consistent follow-up.
• Build awareness of the facility's services, clinical capabilities, and specialty programs.
• Participate in educational presentations, provider meetings, community events, and outreach activities.
• Maintain accurate referral and marketing activity in the appropriate CRM/EHR systems.
• Identify barriers to referrals and communicate trends and opportunities to facility leadership.
• Collaborate with the interdisciplinary team to support census development and successful transitions.
