Zusammenfassung

Strive Health is seeking a Clinical Lead Nurse Practitioner to guide care for CHF, CKD/ESKD and other chronic conditions within a hybrid-remote model, including home visits and telehealth. You will lead a patient panel with advanced analytics and interdisciplinary collaboration to deliver longitudinal, patient-centered care.

The role requires NP board certification, 2+ years in cardiology or nephrology, and proficiency with EHRs, along with a commitment to reducing hospitalizations and advancing

Qualifikationen

• Master’s degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner (NP), Primary Care NP or equivalent with specialization in cardiology (CHF) or Nephrology (CKD).
• 2+ years experience with Electronic Health Records (EHR).
• Current state specific licensure.
• National Board Certification.
• Current DEA license or eligible to obtain within 90 days of hire.
• Current BLS or CPR Certification.

Aufgaben

• Manages a defined patient panel with accountability for quality, cost and outcomes.
• Obtains patient history, performs physical exams, orders and interprets diagnostic tests.
• Ensures assessment and plan of care incorporate best practices for CKD/ESKD and heart failure; supports transitions in care.
• Documents and captures diagnoses to support risk adjustment (HCC coding) and identify care gaps.
• Presents patient cases and provides clinical support for rounds and interdisciplinary meetings.
• Delivers in-person and telehealth patient visits across multiple settings including clinics and homes.
