Job summary

Strive Health is seeking a Clinical Lead Nurse Practitioner to guide care for CHF, CKD/ESKD, and other chronic conditions. You will provide longitudinal, home- and telehealth-based care with an emphasis on high-touch, whole-person approaches.

Leverage analytics and team collaboration to reach patients where they are. You’ll balance autonomy with evidence-based practice, maintain board certifications, and travel regionally to meet patient needs while reducing hospitalizations and improving

Qualifications

• Master’s degree in Nursing or similar field.
• 2+ years experience as a Family NP, Primary Care NP or equivalent with specialty in cardiology (CHF) or nephrology (CKD).
• 2+ years experience with Electronic Health Records (EHR).
• Current state licensure; National Board Certification.
• Current DEA license or eligible to obtain within 90 days of hire.
• Current BLS or CPR Certification.
• Willingness to travel across an assigned region to meet patient needs.

Responsibilities

• Manages a defined patient panel with accountability for quality, cost and outcomes.
• Obtains patient history, performs physical exam, orders and interprets diagnostic tests and formulates a plan.
• Addresses CKD/ESKD and heart failure management, including transitions and shared decision-making.
• Documents diagnoses for risk adjustment and addresses care gaps.
• Provides clinical support for rounds and interdisciplinary meetings.
• Delivers patient visits in multiple settings including home and telehealth.
