Strive Health Services LLC seeks a Family Nurse Practitioner to lead clinical care for CHF, CKD/ESKD, and other chronic conditions. You’ll deliver longitudinal, home-based and telehealth care, supported by advanced analytics and collaborative care teams.

In this hybrid role, you will manage a defined patient panel, ensure high-quality outcomes, and travel as needed across offices, clinics, and patient homes, while maintaining board certification and licensure.

Formación

• Master’s degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner (NP), Primary Care NP or equivalent with specialization in cardiology (CHF) or Nephrology (CKD).
• 2+ years experience with Electronic Health Records (EHR).
• Current state licensure.
• National Board Certification.
• DEA license or eligible within 90 days; current BLS/ CPR certification.
• Mobility with active driver’s license for regional travel across offices, clinics, and patient homes.
• Proficiency in computer usage and Microsoft Office tools.
• Reliable internet connectivity.
• Willingness to travel and be on-site as needed.

Responsabilidades

• Manages a defined patient panel with accountability for quality, cost and outcomes; collaborates with external providers and care teams.
• Obtains patient history, performs exams, orders and interprets tests, and formulates short-term and longitudinal care plans.
• Incorporates CKD/ESKD and heart failure best practices into assessment and care planning; supports transitions as needed.
• Documents diagnoses to support risk adjustment (HCC coding) and addresses care gaps across preventive care.
• Presents cases and provides clinical support during rounds and interdisciplinary meetings.
• Provides care across multiple settings including home visits, telehealth, and clinics; travel as required.
• Maintains current board certification and state CE requirements; adheres to documentation policies.
• Delivers in-person care as needed and engages with stakeholders to drive team and business priorities.
