Job summary

Catholic Health Services is seeking a Clinical Intake Case Manager in Florida to perform front line clinical assessments and facilitate patient placement in the appropriate level of care. You will collaborate with physicians, case managers, and insurers to streamline the admission process.

Responsibilities include coordinating inpatient admissions, communicating status updates, and educating stakeholders on facility programs while maintaining compliance with relevant laws and Life Care standards.

Qualifications

• Active RN license to practice in Florida, 3-5 years of experience in a health care facility.
• Or other license such as LPN/Therapy with minimal 10+ years of experience in a Health Care Facility required
• Active Nurse Practitioner (APRN) license to practice in the state of Florida preferred
• Experience in Marketing and Admissions
• Working knowledge of the patient referral process for Rehab hospital and Skilled nursing facility
• Knowledge of Medicare, Medicaid, Managed Care, and other private insurance
• Must be able to understand and effectively navigate managed care process
• Must be able to function in a fast paced environment.
• Excellent customer service skills
• Must have excellent verbal and communication skills
• Must have a valid Florida driver's license
• Multilingual preferred
• Must have knowledge of computer office/clinical software
• Must be able to read, write and understand the English language

Responsibilities

• Perform front line clinical assessments to determine eligibility and placement in appropriate level of care.
• Establish and maintain relationships with providers, referral sources, physicians, facilities and the community.
• Discuss cases with physicians and clarify orders.
• Educate physicians, case managers, and insurers on facility programs.
• Coordinate all inpatient admissions and referral-to-admission processes.
• Ensure referrals are entered timely and complete before admission.
• Communicate changes in patient status to scheduling and admissions teams.
• Serve as primary contact between participants and CHS team.
• Ensure proper documentation and discharge documents are uploaded.
• Support scheduling and admission processes with appropriate communications.
