Job summary

Catholic Health Services is seeking a Clinical Intake Case Manager to perform frontline clinical assessments, determine eligibility for the appropriate level of care, and help increase census while complying with laws and Life Care standards.

You will coordinate admissions, communicate with physicians, referral sources, and families, and ensure timely documentation and seamless transitions across the continuum of care. Strong communication and customer service skills are essential.

Qualifications

• Active RN license to practice in the state of Florida.
• Experience in marketing and admissions and patient referral process.
• Knowledge of Medicare, Medicaid, Managed Care, and private insurance.
• Excellent verbal and communication skills.

Responsibilities

• Perform frontline clinical assessments to determine eligibility and level of care.
• Coordinate all inpatient admissions and manage referral-to-admission process.
• Communicate with physicians, referral sources, and families for seamless transitions.
• Ensure timely, accurate submission of discharge documents and changes in status.
• Adhere to policies, laws, and Life Care standards.
