Qualifications

Responsibilities

• Evaluates physician medication orders for appropriateness of drug and dosage.
• Maintains patient/resident database including current and past medical history.
• Ensures appropriate laboratory tests are ordered and evaluated.
• Collaborates with healthcare team to provide clinical support.
• Documents all communication in the medical record.
• Implements policies for drug storage and disposal.
• Maintains required documentation for regulatory compliance.
• Informs the Pharmacy Manager of medication-related errors.
• Assists facility staff with drug-related procedures.
• Performs other tasks as assigned.
