Job summary

VITAS Healthcare in Chicago, IL seeks a Team Manager to lead the Patient Care Team, supervise, evaluate and coordinate interdisciplinary staff, ensure continuity of care from admission to discharge or bereavement, and act as patient advocate.

The role is office-based with a Mon-Fri 8am-5pm schedule and one field day per week; salary ranges from $95k to $110k depending on experience. The Team Manager will oversee scheduling, quality improvement, regulatory compliance and staff development.

Qualifications

• Two years of supervisory experience or equivalent in a health care organization.
• Knowledge of primary medical and nursing care principles and practices.
• Understanding of interpersonal dynamics within an interdisciplinary health care team.

Responsibilities

• Directs patient care services for the team and ensures integration with the interdisciplinary care plan.
• Reviews admissions, history and discharges to ensure eligibility and appropriateness.
• Collaborates with team and attending physicians on palliative care measures.
• Maintains accurate patient records from intake to discharge.
• Manages team scheduling to ensure 24/7 coverage.
• Participates in on-call rotation.
