Job summary

Adult & Child Health in Indianapolis/Greenwood is seeking an Evaluator to perform comprehensive initial assessments and diagnostic evaluations for behavioral health services. The role involves developing treatment plans, provisional diagnoses, and referral recommendations, with occasional brief therapy and safety planning.

Clinicians may provide supervision to trainees pending approval. The position requires a Master’s degree in Social Work, Psychology, Counseling, or a related field and active

Qualifications

• Master’s degree in Social Work, Psychology, Counseling or related field.
• Active state licensure required (LP, PLP, LPC, PLPC, LCSW, LMSW, LMFT, PLMFT, or equivalent).
• Background checks including fingerprinting are required.

Responsibilities

• Conduct comprehensive screenings and initial assessments for individuals seeking services.
• Determine appropriateness and eligibility for services based on clinical findings.
• Develop treatment plans, provisional diagnoses, and referral recommendations.
• Provide brief therapy sessions and safety planning when clinically necessary.
• Maintain accurate documentation within electronic health record systems.
• Engage in crisis intervention services as needed.
