Job summary

Adult & Child Health seeks an Evaluator to perform comprehensive screenings and initial assessments for clients of all ages in Indianapolis/Greenwood. The role involves establishing provisional diagnoses, developing treatment plans, and making referrals aligned with program capabilities.

The evaluator may provide brief therapy and safety planning as needed, while documenting thoroughly in the electronic health record and supporting quality initiatives.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field.
• Active state licensure required (LP/PLP/LPC/PLPC/LCSW/LMSW/LMFT or equivalent).
• Experience in behavioral health assessments and crisis intervention desirable.

Responsibilities

• Conduct comprehensive screenings and initial assessments for clients across the lifespan.
• Determine service eligibility based on clinical findings.
• Develop treatment plans and provisional diagnoses; make referral recommendations.
• Provide brief therapy and safety planning as needed.
• Maintain compliant documentation in electronic health records and associated systems.
