Job summary

Adult & Child Health in Indianapolis/Greenwood is seeking an Evaluator to perform comprehensive assessments and provisional diagnoses for clients seeking behavioral health services. The role involves treatment planning, referrals, and safety planning as needed, with potential supervision of trainees pending approval.

The ideal candidate holds a Master’s degree in Social Work, Psychology, or Counseling and active licensure in the relevant state.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field is required.
• Active state licensure required (LP/PLP/LPC/PLPC/LCSW/LMSW/LMFT/PLMFT, or equivalent).

Responsibilities

• Conduct comprehensive screenings and initial assessments for individuals seeking services across the lifespan.
• Determine eligibility for services based on clinical findings.
• Develop treatment plans, provisional diagnoses, and referrals as appropriate.
• Provide brief therapy sessions and safety planning when needed.
• Maintain accurate documentation in electronic health record systems.
• Participate in crisis intervention services as needed.
