Job summary

Adult & Child Health is hiring an Evaluator in Indianapolis. The role conducts comprehensive screenings and initial assessments across the lifespan, determines service eligibility, and develops treatment plans with provisional diagnoses.

You may provide brief therapy and safety planning as needed, while ensuring documentation in the EHR and coordinating referrals. The evaluator will participate in collaboration with teams, uphold professional ethics, and support quality improvement initiatives

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field.
• Active state licensure required (LP, LMSW, LPC, PLPC, LCSW, LMFT, etc.).
• Background checks and required trainings completed at start.

Responsibilities

• Conduct comprehensive screenings and initial assessments.
• Develop treatment plans, provisional diagnoses, and referral recommendations.
• Provide brief therapy and safety planning when clinically necessary.
• Maintain accurate documentation in the EHR and ensure follow-up.
