Job summary

Verrazano Nursing & Post-Acute Center in New York seeks a Dietitian to assess residents' nutritional needs, develop individualized meal plans, and ensure compliance with federal and state regulations. The role requires collaboration with nursing staff, physicians, and food service to promote health through proper nutrition.

The Dietitian will monitor care plans, educate residents and families, review menus for nutritional adequacy, and maintain accurate documentation.

Qualifications

• Bachelor’s degree in Nutrition, Dietetics, or a related field is required.
• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
• 1–2 years of experience in long-term care or clinical settings preferred.
• Knowledge of MDS 3.0, care planning, and CMS regulations for nursing homes.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Assess residents' nutritional status and dietary needs.
• Develop and monitor individualized nutrition care plans.
• Collaborate with physicians, nurses, speech therapists, and food service staff.
• Conduct regular nutritional assessments and update care plans.
• Monitor food service operations for dietary adherence and resident satisfaction.
• Educate residents, families, and staff on nutrition topics.
• Maintain documentation per CMS/state regulations.
• Participate in care plan and interdisciplinary meetings.
• Review menus to meet residents' nutritional needs.
• Stay updated on geriatric nutrition best practices.
