Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing or eligible to practice.
• Knowledge of state survey process and nutrition regulations is preferred.
• Strong organization and communication skills.
• Ability to interact with residents, families, staff and leadership.

Responsibilities

• Assess clinical nutrition needs of residents/patients and document plans in the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical data and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health and well-being.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs.
