Job description

Responsibilities

1. Performs nutritional assessment using age and gender appropriate standards, considering cultural, religious, and ethnic backgrounds; utilizes validated assessment tools to diagnose nutrition-related issues when indicated. Conducts routine screening to identify patients at nutritional risk and collaborates within an interdisciplinary team to facilitate care and identify patients needing higher levels of care.
2. Implements targeted nutrition interventions and provides customized nutrition education to patients, families, and caregivers. Monitors, evaluates, and re-screens patient nutrition status regularly, updating the care plan as needed and following up with patients.
3. Develops an intervention plan including appropriate feeding routes, modes, and nutrition prescriptions tailored to patient needs, in accordance with physicians' orders. Monitors patient response to interventions and documents progress in the medical record.
4. Collaborates with Food and Nutrition Services to enhance patient experience through participation in tray testing, menu reviews, accuracy audits, and ensuring menu compliance with relevant manuals. Supports staff training and maintains communication with patients and staff to provide excellent service.
5. Performs other duties as assigned by the supervisor.

Qualifications

Required Education and Experience

Bachelor's Degree from an accredited dietetics program (ACEND).

Required Licensure and Certifications

Registered Dietitian (RD) within 3 months; until then, documentation must be signed off by a registered and licensed dietitian. Licensures required include AR, GA, IA, KS, KY, MN, NE, NV, ND, OH, TN, TX, UT, WA, all within 3 months of hire.
