Responsibilities

• Develops an intervention plan tailored to each patient's needs, including feeding route, mode, and nutrition prescription, in accordance with physicians' orders.
• Monitors and evaluates patient responses to interventions, documenting progress in the medical record and meeting specific measurable goals.
• Updates the care plan as necessary and follows up with patients.
• Collaborates within an interdisciplinary team to facilitate care and identify patients at nutrition risk or requiring higher levels of care.
• Works with the Food and Nutrition Services team to enhance patient experience.

Qualifications

Required Education

• Bachelor's degree
• Completion of an accredited dietetics program (ACEND)

Required Licensure & Certifications

• Valid registration as a Registered Dietitian with the Commission on Dietetic Registration (CDR), within 3 months of hire
• Dietitian State license, within 3 months of hire
