Job summary

Fresenius Medical Care seeks a skilled Social Worker to provide psychosocial services for dialysis patients in our facility. You will assess psychosocial status, plan care with the interdisciplinary team, and offer counseling to patients and families.

You will educate patients about treatment options, assist with advance directives, and connect them with community resources and financial assistance as needed. Travel within the clinic network may be required.

Qualifications

• MSW degree required and state social work license.
• 2–5 years of related experience in psychosocial oncology? (Note: based on the ad, experience in dialysis/social work is implied)
• Strong counseling, assessment, education and care coordination skills.

Responsibilities

• Assess patients' psychosocial status and plan care in collaboration with the care team.
• Provide counseling and education to patients and families; support adherence and quality of life.
• Coordinate with Insurance Coordinators for financial information and patient resources; facilitate referrals.
