Your Health Organization in Andrews is seeking a Clinical Coordinator to support patient care in our clinic. You will handle chart prep, scheduling, care management logging, and provider and patient support to ensure smooth coordination of care.

This full-time, 8-hour day role (8:00 AM–5:00 PM) Monday–Friday emphasizes accurate documentation, privacy compliance, and collaboration within a value-based care team.

Qualifications

• Must hold CMA or nursing license and be in good standing.
• Minimum one year in a healthcare setting preferred.
• Strong communication, organization, and confidentiality.

Responsibilities

• Review and organize patient medical records for visits.
• Ensure documentation, test results, and diagnoses are accessible for the provider using Navina.
• Reconcile medications.
• Schedule and coordinate patient appointments with providers and care team.
• Communicate with patients via email, portal, phone, text, and in person.
• Log care management activities accurately and securely.
• Maintain confidentiality and privacy compliance.
