Job summary

Your Health Organization in Savannah is seeking a Clinical Coordinator to support clinical operations for our patient population. This full-time role focuses on chart prep, scheduling, documentation, and care management coordination to ensure seamless patient care.

The coordinator will use Navi­na to manage records, communicate with teams via AthenaText and Microsoft Teams, and maintain accurate patient information across facilities and visits.

Qualifications

• Certified Medical Assistant (CMA) or licensed nurse (LPN/RN) in good standing.
• Current CPR certificate.
• High school diploma or equivalent; additional healthcare coursework is a plus.
• Strong communication and interpersonal skills in a fast-paced care setting.
• Must be able to drive and have reliable transportation.

Responsibilities

• Review and organize patient records for visits.
• Ensure documentation and test results are accessible to providers.
• Coordinate patient appointments with care team and facilities.
• Maintain accurate patient demographics and contact details.
• Communicate with patients via email, portal, phone, or text to schedule and prep visits.
