Job summary

Sevita is seeking a Clinical Coordinator for IDD services in Idaho Falls, ID. This full-time role focuses on delivering community-based support in residential, vocational, or in-home settings.

The position requires a Bachelor’s in a human services field and at least two years of related experience, with training in behavior modification or behavior management. You will work with a team to ensure compliant, person-centered services and advocate for participants’ rights.

Qualifications

• Bachelor’s degree in a human services field is required.
• Minimum of two years of related experience.
• Training in behavior modification techniques or experience with behavior management required by state or program funder.
• Knowledge of populations served (IDD, brain injury, mental health) and expertise in behavioral support, early intervention, or crisis intervention.
• Reliable, responsible, caring, and collaborative team player.

• Valid driver’s license, car registration, and auto insurance if transporting individuals.
• All state-required training and certifications must be completed in mandated timeframes.

Responsibilities

• Ensure services adhere to Individual Plans, quality standards, and regulatory requirements.
• Complete structured assessment interviews with parents/care providers.
• Conduct observations in home, school, or community settings.
• Write and submit assessment summaries and recommended behavior plan components to IDT or operations leader.
• Advocate for rights of individuals and present behavior plan information to review committees.
• Document progress, maintain confidentiality, and complete billing documentation.
• Maintain professional relationships with individuals, families, guardians, and case managers.
• Attend staff and interdisciplinary team meetings as needed.
• Assist with activities of daily living and transportation as needed.
• May accompany individuals to medical appointments and relay instructions.
