Your Health Organization in Walterboro, SC is seeking a Clinical Coordinator to support all clinical needs for our patients. You will prepare charts, coordinate patient visits, and log chronic care management activities in a fast-paced, value-based care environment.

The role requires CMA or nursing licensure, CPR certification, and strong communication skills. This is a full-time, on-site position with hourly support for collaboration across care teams and facilities.

Qualifications

• Must be a CMA or licensed nurse (LPN/RN) in the state of employment.
• Minimum of 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure documentation, test results, and diagnoses are accessible for the provider; use Navina to update records.
• Maintain confidentiality and privacy; verify active consent forms.
• Reconcile medications and coordinate care with the provider and care team.
• Schedule and coordinate patient appointments across offices and facilities.
• Communicate with patients via email, portal, phone, text, and in person; provide pre-appointment instructions.
• Document and log care management activities accurately.
