Job summary

Your Health in Savannah is seeking a Clinical Coordinator to support patient care across facilities. You will prepare charts, schedule visits, and log care management activities to ensure seamless operations.

The role requires CMA or licensed nurse, CPR, and at least one year in healthcare. You will work Monday-Friday, 8 hours, in the Savannah office, with some travel.

Qualifications

• Must be a CMA or licensed nurse (LPN/RN) in good standing.
• Minimum of one year of experience in a healthcare setting preferred.
• Must hold and maintain current CPR certification.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and sound judgment.
• Ability to read and communicate effectively.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Must be able to travel between offices and work flexible hours.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation and test results are accessible for the provider; use Navina to update records.
• Maintain accurate up-to-date patient records including demographics, contact details, and insurance information.
• Ensure confidentiality and privacy; verify active consent forms and alignment paperwork in patient charts.
• Reconcile medications.
• Schedule and coordinate patient appointments with the provider and care team.
• Interact with patients via email, portal, phone, text, or in person to gather information and provide pre-appointment instructions.
• Document and log care management activities.
