Job summary

WellPower – Residential in Denver, CO is seeking a Clinical Case Manager I/II to provide comprehensive psychosocial case management within a continuum of services designed to improve functioning and recovery. You will assess and monitor basic needs, maintain clinical documentation, and support clients in accessing community resources.

The role includes transportation for people served in your vehicle (insurance re-imbursement); standard Monday through Friday hours within a defined hiring range.

Qualifications

• Bachelor (CM I) or Master (CM II) degree in psychology, social work, or other related human services degree.
• One year experience in mental health or human services preferred.
• Valid Colorado Driver’s license with a recent clean driving record; ability to transport the people we serve in your vehicle.

Responsibilities

• Provide comprehensive psychosocial services to improve or maintain individuals’ functioning.
• Coordinate transportation for people served using agency vehicle or personal vehicle.
• Help people served establish goals and plans for recovery and daily living skills.
• Assist with identifying and connecting to community resources and benefits.
• Maintain thorough clinical documentation of consumer interactions.
• Support trauma‑informed care and perform other duties as assigned.
