Job summary

Conard House, Inc. in San Francisco is seeking a licensed mental health professional to provide individual and group psychotherapy, case management, and crisis intervention within the Supportive Housing Program.

Requirements include a Master’s degree in Psychology or Counseling, CA BBS registration or eligibility (AMFT/APCC/ASW) or licensure, and at least 2 years of direct client services in a mental health setting. Documentation in Epic and coordination of referrals are essential.

Qualifications

• Master’s degree in Psychology, Counseling, or closely-related field.
• Registration with the CA Board of Behavioral Sciences as AMFT, APCC, or ASW, or licensure.
• Licensed or waivered status preferred.
• 2 years of direct client services experience in a mental health setting.

Responsibilities

• Provide individual and group psychotherapy.
• Conduct psychosocial case management and rehabilitation services.
• Perform diagnostic assessments and crisis intervention.
• Document all services in Epic per guidelines.
• Lead admissions, orientation, and referrals coordination.
• Develop and implement case management and living skills plans.
• Advocate for residents to secure entitlements and healthcare.
• Lead therapeutic groups and structured activities.
• Meet monthly billing targets; promote community engagement.
• Respond to crises per procedures and coordinate with supervisors.
• Develop discharge plans for transitions out of the program.

Ensure all services are documented in Epic in accordance with Conard House documentation guidelines.Conduct psychiatric assessments to determine medical necessity and clinical appropriateness for program eligibility.Offer residents support and resources on a wide range of concerns in a constructive, non-judgmental manner.Facilitate the admission and orientation process for new residents.Assess residents’ needs for medical and psychiatric evaluations and coordinate appropriate referrals.Develop and implement case management plans, money management strategies, and contractual agreements to support resident stability.Assist residents in developing essential daily living skills.Advocate on behalf of residents to secure entitlements, healthcare, mental health services, clothing, and other necessary resources. Licensure hours for LCSW, AMFT, LPCC or Psychology are available.2 years of experience in any occupation or position providing direct client services in a mental health setting preferred.Experience in mental health services, including psychiatric diagnostic assessment, individual and group counseling/therapy, psychosocial case management, and crisis intervention.Demonstrated effective advocacy, organizational, and communication skills.Experience working with diverse populations and diverse teams.
