Conard House seeks a skilled mental health case manager to provide counseling, case management, and care coordination for residents. You will assess needs for medical and psychiatric referrals, document services in Epic, and coordinate Plans of Care with multidisciplinary teams.

The role includes leading group activities, advocating for entitlements, meeting billing targets, and supporting residents in daily living and community resource access within a supportive housing setting.

Formación

• Bachelor's Degree and 4 years of mental health experience required.
• OR AA plus 6 years of mental health setting experience with 2 years post-AA.
• Experience with case management, outpatient billing, and diverse populations.
• Strong assessment, advocacy, organizational, and communication skills with supported housing.

Responsabilidades

• Provide individual rehabilitation counseling and psychosocial case management.
• Assist residents with finances, purchases, and accessing resources.
• Lead groups and activities to promote stable living and community engagement.
• Admit and orient residents, assess needs for medical/psychiatric referrals.
• Coordinate Plans of Care with other services and document in Epic per guidelines.
• Respond to crises and support daily living skills; meet monthly billing targets.
