Job summary

DESC (Downtown Emergency Service Center) seeks intake specialists and case managers to support adults with severe mental illness in their journey toward recovery. SAGE services coordinate housing, clinical care, and community resources within a Recovery-based model.

Role involves multi-disciplinary teamwork, outreach, crisis intervention, and collaboration with housing staff to retain housing and improve stability.

Qualifications

• Bachelor’s degree in social work, psychology, or related behavioral science, OR 1 year of relevant paid work experience with demonstrated ability to perform required duties.
• Ability to drive and possess a valid WA driver's license and insurable driving record.
• Interest in working with clients who are difficult to engage and maintain in traditional mental health/substance use disorder programs.
• Knowledge of Recovery-focused, strengths-based work in mental health and Harm Reduction strategies.

Responsibilities

• Participate as a member of a multi-disciplinary team providing ongoing case management to adults with severe and persistent mental illnesses.
• Collaborate with Scattered Sites Supportive Housing for client care and housing retention plans.
• Provide outreach to assigned clients and maintain working relationships.
• Conduct psychosocial assessments and develop goal planners with consumer participation.
• Coordinate all aspects of consumers' support, including psychiatric and substance abuse treatment, 24-hour support, and housing access.
• Advocate for access to community resources and ensure continuity of care.
• Travel locally for outreach, home visits, and housing related tasks.
• Provide crisis intervention and risk assessment; offer counseling as qualified.
• Fulfill protective payee duties and assist with budgeting and purchases.
• Maintain professional relationships with landlords and document services with deadlines.
