Job summary

TASC (Treatment Alternatives for Stronger Communities) is hiring a Case Manager for the Housing Is Recovery program in the Housing Division. You will provide specialized case management to secure stable, affordable housing and connect clients to essential services, using trauma-informed, person-centered care.

Requirements include a Bachelor’s degree and 2+ years of direct case management. Certification pathways (CADC/CRSS) can be pursued within the first two years.

Qualifications

• Bachelor’s degree from an accredited college or university in social work, psychology, criminal justice, or related field preferred.
• Minimum of 2 years of case management experience providing direct client services.
• Ability to obtain CADC/CRSS certification within the first two years in the field.

Responsibilities

• Collaborate with the Housing Network Developer to identify safe, affordable housing options for reentry clients.
• Provide specialized case management including interviews, intake assessments, and housing stabilization plans.
• Conduct housing screenings and assess social determinants of health to inform plans.
• Navigate service systems, make timely referrals to community-based providers and supports.
• Enter accurate case documentation into the ECR database and collect required documents.
• Support monthly data collection, outcome tracking, and program reporting.
• Develop working relationships with internal and external partners.
