Job summary

DESC (Downtown Emergency Service Center) seeks an experienced case management professional for its SAGE program, focusing on adults with serious mental illness. You will provide outreach, psychosocial assessments, and coordinated care across housing, medical, and support services.

The role is union-represented (SEIU Healthcare 1199NW) and located at DESC's Birch Grove site. The position offers a stable, full-time schedule with a competitive hourly wage and comprehensive benefits.

Qualifications

• Minimum: Registration as a Registered Agency Affiliated Counselor (AAC) or superseding credential.
• Bachelor’s degree in social work, psychology, or related behavioral science, or 1 year of relevant paid work experience with ability to perform duties.
• Valid Washington State driver’s license with insurable driving record.
• Ability to work with clients who are difficult to engage and maintain in programs; knowledge of Recovery-focused, strengths-based work in mental health; knowledge of Harm Reduction.

Responsibilities

• Participate as a member of a multi-disciplinary team providing ongoing case management for adults with severe and persistent mental illness.
• Collaborate with Scattered Sites Supportive Housing in client care and housing retention plans.
• Provide outreach, engagement, and crisis intervention; conduct psychosocial assessments; develop goal planners with consumer participation.
• Coordinate services including psychiatric treatment, substance abuse treatment, 24-hour support, and access to food, housing, medical care, and employment resources.
• Establish professional relationships with landlords and providers to ensure continuity of care and housing retention.
