Job summary

Brightpoint in Chicago is seeking a Clinical Case Manager for Adoption Support and Preservation (ASAP). You will assess behavioral health needs, develop treatment plans, and deliver or coordinate therapeutic interventions in home, community, or office settings.

This full-time, hybrid role requires a Master’s degree or a Bachelor’s degree with relevant experience, plus a valid driver’s license. Fluency in English and Spanish is strongly preferred; Cook County resources knowledge is a plus.

Qualifications

• Master’s Degree in Social Work or related field preferred or Bachelor’s Degree with at least two years of relevant experience.
• Valid driver’s license, insurance, and reliable vehicle required.
• Ability to communicate effectively in English and Spanish strongly preferred.
• Knowledge of Cook County area resources strongly preferred.

Responsibilities

• Maintains a caseload or provides services with another provider to participants.
• Assess participant needs, strengths, and behavioral health functioning.
• Formulates treatment plans and documents progress with supervisory approval.
• Engages participants in individual, family, and/or group therapy as needed.
• Coordinates services and attends meetings with other agencies and community providers.
• Develops aftercare treatment plans and ensures services are in place before discharge.
