Job summary

Brightpoint invites you to join our team as a Clinical Case Manager for the Adoption Support and Preservation (ASAP) program in Chicago. The role provides healing-centered, family systems-based therapeutic services to adopted children and their families, aiming to maintain placement and improve well-being within the home environment.

Qualifications

• Master's Degree in Social Work or Bachelor's Degree in Social Work with at least two years of relevant experience.
• Valid driver's license, insurance, and a reliable vehicle.
• Bilingual English and Spanish strongly preferred.
• Knowledge of Cook County resources strongly preferred.

Responsibilities

• Maintains a caseload or provides services in collaboration with another provider to participants.
• Assesses participant’s behavioral health, strengths, and functioning using a mental health assessment.
• Forms treatment plans with participants and implements plans.
• Engages participants in individual, family, and/or group therapy and referrals as needed.
• Coordinates services and attends meetings with other agencies to meet participant needs.
