Powers Health in East Chicago, IN is seeking a Clinical Case Manager (RN) to oversee daily planning, coordination and monitoring of patient care with physicians and nursing staff. The role emphasizes coordination across the care continuum and discharge planning, ensuring compassionate service and efficient use of resources.

The ideal candidate holds a nursing degree (BSN preferred), an active Indiana RN license, 3–5 years of clinical experience, and telemetry familiarity.

Competenze

• Graduate of accredited school of Nursing; BSN preferred.
• Current Indiana RN license.
• 3-5 years recent clinical experience.
• Telemetry experience required.
• Certification in clinical specialty desirable.

Job Responsibilities

• Interacts courteously and compassionately with patients, family/significant others, and other employees.

Education/Experience

• Graduate of accredited school of Nursing, BSN preferred
• Certification in clinical specialty appropriate to the unit assigned patient population desirable
