NFI North is seeking a full-time Clinical Case Manager for our Pathways Transitional Housing Program in Concord, NH. This program is a community residence serving 16 individuals and includes a day treatment component. We provide intensive clinical, medical, vocational, and residential services to adult men and women with mental health challenges working towards independent living. Our goal is to help individuals successfully transition into the community while maintaining their independence in the least restrictive environment possible.
Responsibilities: Provide case management and clinical services to assigned participants, perform administrative duties, and offer clinical support for participants, including supported employment. Additionally, provide clinical training and support for staff. Responsible for managing all financial information and benefits.
Qualifications: Bachelor’s Degree in Human Services or a related field with at least four years of related experience and/or training. Experience with supported employment activities and working with the participant population is preferred.
