Job summary

Kaleidoscope Family Solutions, Inc. is seeking a Case Manager to provide clinical management and oversight for professionals working with individuals with disabilities, while building relationships with families and individuals on caseload.

The role requires a Bachelor's degree in Psychology or Social Work, 1–3 years of human services experience, a reliable vehicle, and willingness to travel throughout the area to visit, perform quality checks, and provide services as needed.

Qualifications

• Minimum of a bachelor's degree in psychology, social work or related field.
• Willingness to travel or drive throughout the area to visit, perform quality checks, and provide services.

Responsibilities

• Conduct intakes with families/individuals to explain services.
• Provide ongoing supervision and training to unlicensed direct care staff.
• Assign Direct Support Professionals (DSPs) to ensure high-quality services.
• Attend client meetings on caseload (quarterly, yearly, as needed).
• Review staff notes for completion and update as necessary.
• Create and maintain clinically appropriate goals.
• Work with families and teams to initiate, coordinate and facilitate transitions into the program.
• Establish positive relationships and clear communication with families and individuals.
• Demonstrate professional ethics and act as a model for others.
• Supervise unlicensed staff.
