Clinical Care Transition Specialist – Registered Nurse

Job summary

Jobtailor is seeking a healthcare liaison in Fort Wayne, Indiana. This role focuses on building relationships with referrals sources, coordinating patient care transitions, and educating patients, their families, and care facility staff.

The successful candidate will serve as a primary contact for Option Care and collaborate with account management to drive growth. The position requires a licensed RN or Pharmacist, with at least two years in healthcare and strong relationship-building skills.

Qualifications

• Licensed RN or Pharmacist in the state of practice.
• At least 2 years of experience in the healthcare industry.
• Experience establishing and maintaining relationships with individuals at all levels of the organization.
• Experience applying knowledge of standard practices for all services offered and relevant standards.
• Experience providing customer service to internal and external customers.
• Basic PC skills: Internet, Email, Word, Excel, PowerPoint.
• Access to reliable transportation for travel to care facilities.
• Able to plan, organize and present.

Responsibilities

• Reviews the patient’s medical record to obtain pertinent medical history and insurance payor information.
• Proactively initiates care transition coordination with referral sources and internal partners.
