CenterWell is seeking a Clinical Care RN to improve patient outcomes through effective Transitions of Care, post-discharge follow-up, and data-driven quality initiatives. You will engage patients and providers, educate on disease management, and collaborate with care teams to reduce avoidable readmissions and ED utilization.

The role requires in-center clinic work in Arizona, with daily navigation of CMS quality measures and Star ratings.

Qualifications

  • Must hold an active, unrestricted RN license in Arizona.
  • Bachelor's or Associate degree in nursing (BSN/ADN) as required.
  • Minimum 3+ years nursing experience with transitions of care, quality improvement, or population health.
  • Proficiency with electronic health records (e.g., Athena EMR) and data analytics tools.
  • Willingness to complete and maintain Basic Life Support training.

Responsabilités

  • Coordinate transitions of care post-discharge to prevent readmissions.
  • Identify care opportunities and conduct outreach to patients and providers.
  • Educate patients on chronic disease management and preventive care.
  • Collaborate with interdisciplinary teams to implement evidence-based interventions.
  • Document outreach activities and outcomes in the medical record.
