Zusammenfassung

CenterWell Senior Primary Care in Arizona’s Surprise clinic is seeking a Clinical Care Nurse (RN) to improve patient outcomes through TOC support, ED utilization reduction and quality performance enhancement. You will educate patients, coordinate post-discharge care, and collaborate with the care team in a fast-paced, compassionate environment.

The role requires an AZ RN license, ADN or BSN, and 3+ years of clinical experience, with proficiency in Athena EMR and data tools to drive Stars and

Qualifikationen

Must hold ADN or BSN and an active AZ RN license.
At least 3+ years of clinical nursing experience with care transitions or population health.
Experience with EMR and data analytics tools is required.

Aufgaben

Analyze clinical data to identify gaps in care related to Stars and HEDIS.
Coordinate timely post-discharge follow-up for TOC and TCM requirements.
Conduct patient and provider outreach via phone, telehealth, and in-clinic visits.
Educate patients on preventive care and chronic disease management.
Collaborate with interdisciplinary teams to implement evidence-based interventions.
Document outreach, interactions, and outcomes per CMS and policy standards.
