CenterWell Senior Primary Care in the United States seeks a Clinical Care RN (Clinic-based). You will focus on improving patient outcomes through targeted TOC strategies, patient and provider engagement, and data-driven quality improvements in a clinic setting.

The role supports Medicare Advantage Stars initiatives and requires in-center work with a Monday–Friday schedule, 8:00 AM–5:00 PM, with some travel within the market as needed.

Qualifikationen

• Must hold ADN or BSN in Nursing.
• Active, unrestricted RN license in the State of Arizona.
• 3+ years of clinical nursing with exposure to transitions of care, quality improvement, managed care, or population health management.
• Proficiency with electronic health records (e.g., Athena EMR), data analytics tools (e.g., DataHub, Compass Rose, Salesforce HealthCloud), and Microsoft Office Suite.
• Willing and able to complete and maintain Basic Life Support training.

• Support safe Transitions of Care (TOC) and reduce avoidable ED utilization.
• Engage patients and providers and drive evidence-based interventions.
• Balance direct patient education with data-driven quality improvement efforts.
• Contribute to Medicare Advantage Stars ratings and overall quality performance.
