CenterWell in Thousand Oaks, CA seeks a Clinical Care Nurse (RN) to support safe transitions of care, reduce avoidable ED visits, and drive Medicare Advantage Stars and quality performance. You will balance patient education with data-driven quality improvement across transitions of care.

The role requires analyzing Athena EMR/DataHub data, conducting post-discharge follow-ups, and collaborating with an interdisciplinary team to prevent readmissions and improve outcomes.

Formación

• RN license, unrestricted, in applicable state (TX).
• 3+ years clinical nursing experience with TOC/quality roles.
• Proficiency with electronic health records and data tools.
• Willing to complete/maintain Basic Life Support.

Responsabilidades

• Analyze data to identify gaps in care related to Stars and TOC.
• Coordinate post-discharge follow-up to prevent readmissions.
• Educate patients and providers on preventive and chronic care.
• Outreach via phone/telehealth/in-clinic visits to close care opportunities.
• Document outreach and ensure CMS-compliant records.
