Job summary

Strive Pharmacy, a personalized compounding pharmacy, is seeking a Clinical Care Pharmacist in Mesa, AZ to manage patient and provider communications and ensure accurate prescription processing. This onsite role emphasizes tailored, patient-centric care and strict adherence to Strive Pharmacy SOPs.

You will perform pre-verification checks, counsel patients, and guide providers on dosing to achieve optimal individualized therapies, in a collaborative, high-service environment.

Qualifications

• Minimum of Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) Degree.
• Active AZ Pharmacist license.

Responsibilities

• Check entered prescriptions for accuracy and appropriateness.
• Perform calculations to ensure ingredients within a formula are measured and utilized properly.
• Manage inbound and outbound communications with patients and providers.
• Counsel patients on therapy and dosing as part of personalized care.
• Offer guidance to providers regarding appropriate therapy and dosing for optimal individualized care.

Position Overview

The Clinical Care Pharmacist manages patient and provider communications, ensures accurate prescription processing, and performs pre-verification to uphold quality control and compliance according to Strive Pharmacy SOPs.

Work Environment
