Job summary

Penn State Health Milton Hershey Medical Center is seeking a Nurse Case Manager to coordinate interdisciplinary patient care, develop individualized plans, and monitor outcomes within clinical pathways and contracts. This full-time, day-shift role emphasizes discharge planning, data collection on care outcomes, and collaboration with families, PCPs, and the care team to improve quality and costs.

Strong communication and Microsoft Office skills are preferred.

Qualifications

• Two years nursing experience.
• Current RN license valid in PA or across compact state.
• BLS certification prior to end of orientation period.

Responsibilities

• Coordinate interdisciplinary professional care of a select patient population.
• Develop, implement, and evaluate treatment plans based on individualized plans of care.
• Coordinate discharge instructions and transition to appropriate level of care.
• Track outcomes and variances to improve quality and cost outcomes.
• Ensure confidentiality and adherence to patient rights and policies.
