Job summary

Select Medical’s Regency Hospital - Florence, SC is seeking a Clinical Liaison (RN or RT) to advocate for referred patients in the pre-admission process. You will collaborate with clinical teams to assess patient needs and coordinate care plans with physicians and families, ensuring smooth transitions into our LTACH care setting.

In this role you’ll work with discharge planners and hospital staff to remove payer and operational barriers, facilitate patient transfers, and support post-ICU care

Qualifications

• Must hold RN or RT license.
• Ability to perform clinical assessments of medically complex patients.
• Strong communication and relationship-building skills required.

Responsibilities

• Plan continuum of care after ICU or post-surgical stay with clinicians, discharge planners, and families.
• Conduct clinical assessments of referred patients and counsel families for best care decisions.
• Collaborate with hospital case managers and physicians to streamline pre-admission and transfer processes.
