The ideal candidate will hold MA licensure (RN/LMHC/LISW/LCSW) and have 3-5 years of relevant clinical experience, with case management experience preferred.

Qualifications

• 3-5 years relevant experience in clinical health care settings (inpatient, outpatient, or varying levels).
• Case Management experience preferred.
• Active licensure in Massachusetts is required, appropriate to position.
• For Behavioral health position, an RN, LMHC, LICSW or LCSW is required.

Responsibilities

• Assess and evaluate member and family needs with strong communication, clinical knowledge, data, and tools.
• Promote adherence to treatment plans and shared decision-making to improve outcomes.
• Develop and document comprehensive care management plans per criteria and policies.
• Review medications and educate on side effects and management for complex needs.
• Engage members to support seamless care transitions and coordinated care.
