Responsibilities

• Respect patient confidentiality and HIPAA regulations.
• Coordinate with practitioners to schedule clinic appointments for discharged patients.
• Complete documentation within designated time frames with compliance and clarity.
• Coordinate patient demographics and health insurance information across the system.
• Serve as liaison to ensure efficiency of transfer and access processes and optimize workflows.
• Assist in scheduling community paramedicine visits and ambulance services.
