Job summary

Brown University Health in Warwick, RI is seeking a Nurse Care Manager to join the primary care team. You will coordinate care for high-risk patients, support the medical home model, and work with providers and pharmacists to optimize outcomes.

The role requires Rhode Island RN licensure; BSN is preferred; current BLS; and chronic care certifications. Typical schedule is M-F, 8:00–5:00, with a Day shift and a commitment to quality patient care.

Qualifications

• State license to practice as a Registered Nurse in Rhode Island.
• BSN preferred, current BLS certification.
• American Academy of Managed Care Nursing (CMCN) or Diabetes Educator certification preferred.
• Ability to coordinate care in a primary care/medical home setting.

Responsibilities

• Collaborate with care team to identify high-risk patients and coordinate care.
• Set up and implement primary care medical home model and care plans.
• Coordinate actions between providers, pharmacists and staff to improve outcomes.
• Provide education on chronic illnesses and self-management to patients.
• Conduct medication reconciliation and support adherence with the care team.
• Participate in interdisciplinary team meetings, huddles and quality initiatives.
