Job summary

St. Joseph's Center in Scranton, PA is seeking a Clinical Care Coordinator to lead nursing services within ICF programs.

The role provides on-site clinical oversight, supervises nursing personnel, conducts resident evaluations, and guides the interdisciplinary team to deliver person-centered, high-quality care. You’ll participate in policy development, support survey readiness, and help with EHR and eMAR implementation.

Qualifications

• Must have a degree from an accredited program recognized by the State Board of Medicine.
• Must hold a valid Pennsylvania NP license.
• Minimum 2 years supervisory experience.
• 3-5 years of clinical experience in related setting.

Responsibilities

• Provide oversight, supervision, and education to nursing staff, including evaluating clinical competencies and delivering training on disease management, infection prevention, medication safety, and wound care.
• Conduct routine medical evaluations, provide emergency care as needed, and manage medication orders and administration within scope of license.
• Ensure adherence to CMS and Pennsylvania ICF/ID regulations, audit nursing documentation, conduct mock surveys and environmental rounds, and support survey readiness.
• Participate in organizational committees and clinical policy development, working closely with the interdisciplinary team to support person-centered care.
• Assist with recruitment and retention efforts, supervise student nurse experiences, and support implementation of Electronic Health Records (EHR) and eMAR systems.
